Please indicate also if you want us to recommend a curriculum for your student by checking YES under the Curriculum Recommendation Service column. The Testing Service fee is $22 per student; the Curriculum Recommendation Service fees is $18 per student. It often takes 2–3 weeks to grade and return your student’s test results. If ordering the Curriculum Recommendation service, please allow an additional week for receipt of both the test results and recommendation.

    The entire fee must accompany the order form. We do not accept partial payments and there are no discounts for multiple orders.

SHIPPING CHARGES

    1. The prices listed on this form include shipping and handling for shipments to United States addresses.

    2. If your order is being shipped to a foreign address, including Canada, please add an extra $5.00. We reserve the right to charge additional shipping fees to foreign addresses when necessary to cover our mailing costs.

CANCELLATION / REFUNDS

    1. Testing Service: a 60% refund will be granted if the unused testing service materials are returned to Christian Liberty Academy within 60 days of the date of shipment.

    2. Curriculum Recommendation Service: if we receive your cancellation notice before we started the recommendation process, we will send you a full refund. Once our recommendation is completed, we are unable to issue a refund for this service.

    3. If you decide to enroll your student with the Christian Liberty Academy School System (CLASS) within six months after receiving the test result, the $22 testing fee will be deducted from your student’s tuition fee. However, if you enroll your student in a grade level using a different Achievement Testing Level than the one she or he originally took, the $22 testing fee will not be deducted from your student’s tuition fee. Please use the CAT Level Chart below to determine if you are eligible for a refund when enrolling your child with CLASS.
